USE THIS FORM FOR: Use this form to change your address, phone number, e-mail address, or any other piece of personal information that is kept in the USNAAA Atlanta database. (This is the information that will be listed for you in the Atlanta Chapter Roster.) NOTE: This form does NOT
change your USNAAAAMAIL List NEW MEMBERS: Do NOT use this form to join USNAAAA. Instead, go to Become a Member and follow the instructions there. Fill in the following form. (Items with an asterisk * are required!) Navigating the Form: TAB between fields or mouse click
in each field. Submitting the Form: Clicking the SUBMIT button at the bottom of the form will e-mail your updated information to the V.P. of Information Systems of our Chapter, who will update the USNAAAA computer database with your new information.
